Synopsis: In two recent studies funded by AHCPR (now AHRQ) assessing health values (utilities) of patients with HIV/AIDS, we showed that: 1) time tradeoff scores for current health are high, indicating that patients have a strong will to live; 2) half of patients felt that their life was better now than before they were HIV-infected; and 3) certain non-health-related factors such as spirituality and concern and love for one’s children correlated with health values and a sense that life has improved. Health values and spirituality have never been assessed in veterans with HIV/AIDS. The objectives are: 1) to assess health ratings and health utilities of veterans with HIV/AIDS and to compare health values of veterans with those of non-veterans enrolled in a concurrent study funded by NIH; 2) to characterize and assess the extent of spirituality in veterans with HIV/AIDS; 3) to derive a power function relating health ratings to utilities; and 4) to pilot test utility questions in non-veterans that address whether society assigns higher values to health states for veterans than for non-veterans.
Details: At two points in time separated by 12-15 months, we plan to interview 100 representative veterans with HIV/AIDS from the Cincinnati and Pittsburgh VAMCs. In addition to clinical and demographic data, the questionnaire battery includes computer-assisted utility assessment (rating scale, time tradeoff, and standard gamble); a question comparing life now with life before (being aware of) being infected with HIV; the HIV/AIDS-Targeted Quality of Life instrument; the HIV Symptom Index; the 10-item Center for Epidemiological Studies - Depression Scale; three spirituality measures tapping different aspects of spirituality related to our conceptual model--the Functional Assessment of Chronic Illness Therapy Spiritual Well-being-Expanded scale, the Duke Religion Index, and Pargament’s Brief RCOPE scale; a new pilot questionnaire on patients’ experiences with and attitudes towards organized religion; a self-reported adherence measure; the Brief Interpersonal Support Evaluation List, a 12-item measure of social support; Rosenberg’s 6-item global self-esteem measure; and the Life Orientation Test, a 12-item measure of optimism. Finally, for the non-veteran patients with HIV/AIDS, we plan to pilot test utility questions about a hypothetical patient with congestive heart failure, identified either as a 72-year-old male veteran of the Korean War or merely as a 72-year-old male. We will ascertain health values of veterans and, using multivariable models and structural equations models, identify factors associated with their health values and opinions regarding whether their lives have improved since contracting HIV.
